About the partner:
You and Type 2 is an NHS-funded project focused on delivering personalised care for people living with Type 2 diabetes in South West London. It aims to give patients more information about their condition and so more control in effectively managing it. The project combines the clinical pathway with innovative digital technologies to provide each person with their own easily accessible personal plan of care, education and support.
Ultimately the project hopes to scale nationally, and offer remote monitoring and testing technologies to allow the Type 2 diabetes population to complete their annual diabetes reviews at home; and build capacity to provide further support to people with more complex needs.
The challenge:
You and Type 2 required a remote finger stick blood testing service, alongside urine and blood pressure testing capabilities.
The solution:
The You and Type 2 project uses Thriva Solutions’ end-to-end at-home blood testing service. This includes test design and assembly; kit distribution and sample delivery to labs; and analysis and reporting. Patients and their GPs are able to access results via a secure, personalised online platform.
Why Thriva Solutions:
- Track record of delivering at-home testing at scale, including involvement in Covid ‘r’ study.
- A dedicated co-ordinator who supports practice staff with the implementation of the tests, including delivery of test kits, patient onboarding and follow up support.
- Ability to integrate the solution with two other partners providing urine and blood pressure tests.
For certain individuals, remote monitoring and testing for diabetes care provides a valued alternative to face-to-face consultations. Commissioning such a pathway at population level would enable economies of scale, reducing the cost per person, making the intervention more scalable, and ultimately freeing up GP practice resources.”
